2022 Lloyd E. Burton Scholarship Award
The Lloyd E. Burton Scholarship is intended to attract students to careers that will improve the public’s health. The scholarship is in recognition of the late Lloyd E. Burton, Ph.D. for his sustained support of AzPHA and his efforts to encourage interest in professions that improve the public’s health.

Background of AZPHA:
What is AZPHA? We are a membership organization that works to improve the level of health and well-being for all Arizonans through advocacy, education and professional development. AZPHA is a nonprofit organization that has been in business since 1928. Our members include health care professionals, state and county health employees, health educators, community advocates, doctors, nurses, students and faculty, and anyone who is interested in public health issues.
Mission Statement:
A non-profit, professional organization working to improve the health of Arizona's communities through advocacy, education, and professional development.
Vision Statement:
Healthy Communities for all Arizonans.
Eligibility:
1. Applicants should be Arizona students who are currently enrolled in a degree-seeking program related to public health (e.g. nursing, medicine, public health, health administration, etc.).
2. Applicants may not be current employees of the Arizona Public Health Association
3. Students must also be current members of AZPHA (Student memberships are $25 and you may become a member online at www.azpha.org)
Application Requirements:
1. Be a current member of AZPHA, preference will be given to active and involved members.
2. Identify current public health activities and affiliations
3. Submit a completed application:
-Provide a 1 page resume
-Official transcripts
-Two letter of support
Scholarship terms and conditions:
Current employees of the Arizona Public Health Association are NOT eligible to apply.

The form will submit electronically. Please submit all supporting documents by mail. All documents must be mailed and postmarked no later than 5:00 pm on Friday, September 27, 2019 to: (incomplete applications or late submissions will NOT be reviewed)
